TISD Athletic Paperwork Step by Step Instructions
|We would like to thank you for taking part in our process of accepting athletic participation paperwork online. This automated process will allow us to be more efficient in handling the forms as well as saving valuable resources.
The following electronic participation forms are required to be completed online each year

* You must have your student’s ID number available as it is required on each form
***Your Child’s LEGAL NAME (as used during registration- must include hyphens if applicable)

Required Paper Forms
All students participating in UIL athletic activities will be required to complete the UIL Medical History Form. Any “yes” answer to any of questions 1-6 on the Medical History Form will require a student to receive further medical evaluation, which may include a physical exam, and receive written clearance from a physician, physician assistant, chiropractor, or nurse practitioner before any participation in UIL practices, games, performances, or matches. Only those students with “yes” answers to those questions are required to receive written clearance through further medical evaluation.
Any participant in athletics, in any grade level, who has not previously completed a Pre-Participation Physical Evaluation (PPE), and been cleared for participation, will be required to complete the Medical History Form as noted above, as well as a PPE prior to participation in any UIL practices, games, performances, or matches.
Please note that this is only for the 2020-21 school year, and local school district policies still apply.

Drop off / Turn in times for Medical History are listed below:
Student athletes that need to be cleared can turn in doctor notes during these dates for clearance.
Times are 7am-8am on the dates below, an AT will be stationed outside the garage door of the TAC.
All online paperwork, hard copy of medical history turned into the athletic trainer, and XLNTBRAIN baseline (HS athletes only) will need to be completed prior to attending Summer Strength & Conditioning
