TISD Athletic Paperwork Step by Step Instructions

We would like to thank you for taking part in our process of accepting athletic participation paperwork online. This automated process will allow us to be more efficient in handling the forms as well as saving valuable resources. 

The following electronic participation forms are required to be completed online each year

  • TISD Forms Signature
  • UIL Forms Signature
  • Athletic Training/Insurance Information

  1. Click on the form name & fill out the information requested. 

* You must have your student’s ID number available as it is required on each form

***Your Child’s LEGAL NAME (as used during registration- must include hyphens if applicable)

*** Emergency Contact phone numbers, Primary Physician, Insurance Policy Information

  1. Do not leave any blank responses.  Use “NA” when necessary or if you do not know the information.
  2. Once you have filled out all of the information on each page you will have the opportunity to print.

Required Paper Forms

  1. All student athletes must complete the UIL Medical History Form & return it to the athletic trainer.
  2. Per UIL: To support social distancing and assist with the workload of the medical professionals on the frontlines of the COVID-19 pandemic, the UIL Medical Advisory Committee (MAC) has recommended adjusting requirements related to preparticipation physical examinations for the 2020-21 school year.

Medical History and Physical Form

All students participating in UIL athletic activities will be required to complete the UIL Medical History Form. Any “yes” answer to any of questions 1-6 on the Medical History Form will require a student to receive further medical evaluation, which may include a physical exam, and receive written clearance from a physician, physician assistant, chiropractor, or nurse practitioner before any participation in UIL practices, games, performances, or matches. Only those students with “yes” answers to those questions are required to receive written clearance through further medical evaluation.

Any participant in athletics, in any grade level, who has not previously completed a Pre-Participation Physical Evaluation (PPE), and been cleared for participation, will be required to complete the Medical History Form as noted above, as well as a PPE prior to participation in any UIL practices, games, performances, or matches. 

Please note that this is only for the 2020-21 school year, and local school district policies still apply.

  *UIL Pre-Participation Physical may be required based upon answers on the UIL Medical History each year.

XLNT Brain Baseline Testing
Temple High School athletes need to use the link below to register & begin baseline test. Once registered, the athlete will be instructed on how to complete the baseline test..  Please allow 30-40 minutes to complete the registration and administration of the test.

TISD Athletes for the 2020-2021 school year can pick up hard copies of the UIL Medical History outside the TAC (old Edwards building) Front door facing Barton Street. 

***Packets will be located in a plastic tub – Please practice social distancing when on campus.

Drop off / Turn in times for Medical History are listed below:

All online paperwork, hard copy of medical history turned into the athletic trainer, and XLNTBRAIN baseline (HS athletes only) will need to be completed prior to attending Summer Strength & Conditioning
***All paperwork will need to be turned in by the athletes or their guardian at the TAC (old Edwards building-Front door facing Barton Street) Map
